About the Program

The purpose of the Life Long Care Program is to assist vulnerable, frail, elderly, Aboriginal People with disabilities and chronic illness (all ages), who have a diminished ability to function independently, and who require care to maintain in their homes. Our services would ensure quality of care or referral to appropriate support services.
